Dental Hygienist Requirements in Hawaii

To become a dental hygienist in Hawaii, you must obtain a license through the Hawaii Board of Dental Examiners. Applicants must be at least 18 years of age to become a dental hygienist in Hawaii. To receive a license, you will need to submit the following information:

Completed Application – An application should be completed using a dark colored pen. Be sure to fill out all required information and sign the form.

Social Security Number – Don’t forget to include a Social Security Number on the application form. You will not receive your license without a SSN.

Fees – There are a variety of fees required including an application fee, a license fee and a compliance resolution fee. For those filing in even years, make a check or money order payable to “Commerce & Consumer Affairs” in the amount of $160. Those filing in odd years should make a check or money order in the amount of $105. Include the check or money order with the application.

Proof of Education – All applicants must have a dental hygiene degree from a program that is accredited by the American Dental Association. Provide proof that you have received this degree by attaching a copy of your diploma or official transcript to the application. Local Anesthesia Certification – In Hawaii, dental hygienists have the duty of applying anesthesia and must be certified in administrating it. Provide a copy of this certification with your application.

National Board Exam – Your National Board Exam scorecard can be sent directly from the American Dental Association to the Board of Dental Examiners. An official scorecard may also be attached to the application.

Examinations – You must successful pass one of the three exams that the Hawaii Board of Dental Examiners will accept:

  • Regional Exam
  • American Board of Dental Examiners Dental Hygiene Exam
  • Central Regional Dental Testing Services Dental Hygiene Exam
